The purpose of this form is to gather necessary information and obtain approval for your child to participate in program services including a bi- annual program evaluation. This consent will remain in effect during the time your child is enrolled in Orange High School.   If services are discontinued and start again at a later date, Family Connections will have access to The Space’s records. In addition, this a joint consent form of FAMILY Connections, Inc. and its clinical staff.
