Vaccine Requirements

  • Attention parents/ guardians,

    • Kindergarten students must have an updated immunization record and a complete physical form on file for Sept 2024.
    • Sixth grade students must have received their ten-year-old  immunizations (Tdap and Meningococcal vaccines) for school entry in September 2024.
