    Orthopedic Conditions / Activity Restrictions

    If your child needs to be restricted from PE for any reason, the Health Office requires a note from a physician.  If an injury has been sustained, that requires crutches, a cast, splinting or any other supportive device, please have your child report to the health office on return to school.  A doctor's note explaining the nature of the injury and necessary restrictions is required. When the injury has completely healed, another note is required, stating that the child is cleared to fully participate in PE/sports. 

    Allergic Conditions
     
    If the allergy requires the use of an Epinephrine auto-injector (Epi-pen, Auvi Q), use the medication administration form. On occasion, both might be applicable. The doctor may order necessary medications on these forms or use a separate medication administration form. Please bring the completed form(s) and necessary medication(s) to the nurse's office. We request 2 auto-injectors to be kept in the health office.  One will be sent to the lunchroom each day and returned to the nurse's office at the end of lunch. In the event of a field trip, teachers will carry the auto-injectors.

    For those students that have ordered Benadryl, Benadryl will also be kept in the nurse's office. However, Benadryl does not go to the lunchroom or on field trips. NJ state regulations allow only an RN, parent who, or a parent designee to administer Benadryl. The parent designee could be another adult, but not a district employee.
     
    Please check with the nurse for any further information (specifics regarding snacks, lunchroom activity, special celebrations, field trips, etc.).
     
    Emergency Contacts
     
    The school district must have accurate emergency contact information on record for all students.  Please be sure to review and if necessary revise the Student Data Verification sheet.  You must update this information.  If you need an extra copy of the sheet, please contact the school secretary.  If you move, change jobs, change phone numbers, change email address, change caretakers, etc., you MUST notify the school secretary with the new information.
     
    Recess/Clothing Requirements
     
    All children go out for recess, weather permitting. Please send them inappropriately dressed. In the winter months, we hold outdoor recess if the temperature (including wind chill) is 20 degrees or higher. Often, grounds can be wet or muddy. Despite our best efforts, children do get soiled clothes from time to time. You may want to keep an extra pair of pants in school for that purpose, especially if you are not available during the day to come in with fresh clothes.
